Job summary

Pine Park Health is seeking a Certified Medical Assistant to serve as the remote primary care coordinator for geriatric patients across California, Nevada, and Arizona. You will manage real-time triage, documentation, scheduling, and cross-functional communication to ensure timely care and patient safety.

You will work independently in a HIPAA-compliant home workspace, leading onboarding and participating in on-call duty, with a focus on quality and efficiency in a high-volume setting.

Qualifications

• 2+ years of direct patient care in a clinical setting.
• Bachelor's degree strongly preferred.
• Experience in healthcare coordination or clinical operations.
• Strong triage and prioritization in real-time environments.
• Proficient with EMR/EHR systems and healthcare platforms.
• Exceptional communication with patients, families, physicians, and staff.
• Self-directed and able to work independently in a fully remote setting.
• Reliable high-speed internet and HIPAA-compliant home workspace.
• Must be in good standing with OIG, CMS, and Federal Health Care Programs.

Responsibilities

• Serve as the primary point of contact for patients, families, facilities and providers across your assigned patient panel.
• Triage urgent care requests and time-sensitive communications in real time.
• Maintain accurate, compliant patient documentation within the EMR system.
• Proactively manage appointment scheduling and follow-up care.
• Lead new patient onboarding with contact and scheduling within 24 hours of enrollment.
• Participate in after-hours on-call rotation.
• Contribute to quality improvement and operational efficiency projects.
