Job summary

Humana Healthy Horizons in Indiana is seeking a Care Coordinator 2 (Field Care Manager 2) to assess and evaluate members’ needs to guide them toward appropriate care resources. This hybrid role requires travel to meet members and manage health outcomes within Indiana’s PathWays program.

The position emphasizes coordination with the care team, Medicare plans, and providers, with on-site and remote work options and travel up to 70% of the time.

Qualifications

• Licensed Registered Nurse in Indiana without restrictions.
• At least two years of clinical nursing experience in case management or similar health care.
• Strong communication and interpersonal skills with internal/external stakeholders.
• Ability to manage multiple priorities in a fast-paced environment.
• Experience with electronic medical records and standard software programs.

Responsibilities

• Assess and evaluate member needs to guide care decisions.
• Coordinate medical and non-medical services to ensure continuity of care.
• Develop a longitudinal, trusting relationship with each member.
• Collaborate with ICT and other care team staff to address identified needs.
• Coordinate with Medicare payers and providers for relevant benefits.
