Qualifications

• Licensed Registered Nurse (RN) in Indiana with no restrictions.
• At least 2 years of clinical nurse experience in case management or similar
• Proficient in Word, Excel, and Outlook
• Excellent communication and interpersonal skills
• Lives in Indiana and available for 40-hour week; travel up to 70% may be required

Responsibilities

• Facilitate longitudinal, trusting relationship with each member toward improved care.
• Coordinate all medical and non-medical services for the member.
• Collaborate with Service Coordinator, Transition Coordinator, and care team.
• Coordinate with Medicare payers and providers for Medicare beneficiaries.
• Serve as primary contact for Interdisciplinary Care Team (ICT) and coordinate with member and resources.
