Qualifications

• RN with minimum of 3 years direct patient care.
• Current active RN licensure; Compact license preferred.
• Must hold at least one of CCM, CRRN, CDMS, COHN, CRC or be eligible.

Responsibilities

• Assess and analyze an injured employee’s medical and vocational status to determine needs.
• Develop a plan of care to facilitate return to work and coordinate with the employer.
• Interview patients in various settings to provide ongoing case management.
• Communicate with patients, providers, adjusters, attorneys, and vendors.
• Coordinate and monitor referrals, treatments, and services for patients.
• Ensure transportation and translation as needed for appointments.
