Job summary

ForzaCare is seeking a Field Medical Case Manager in the United States (Dodge City, KS). You will serve as the central coordinator among injured workers, medical providers, employers, and insurers to ensure timely and effective care management.

Ideal candidates hold an active RN or CRC license with a college degree. Prior workers' compensation case management experience is highly preferred, and travel to appointments is required as needed.

Qualifications

• Active RN or CRC license with associated college degree is required.
• CCM, CIRS, or other case management credentials are preferred.
• Prior experience in workers' compensation case management is strongly preferred.
• Must have valid driver's license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to employers and workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
