Job summary

ForzaCare in the United States is seeking a Field Medical Case Manager to coordinate care for injured workers, connect them with providers, employers, and insurers, and ensure timely, transparent care management.

The role is ideal for licensed nurses (RN) or certified rehabilitation counselors (CRC). Prior workers' compensation experience is strongly preferred, with willingness to travel to appointments across clients' sites.

Qualifications

• Active RN or CRC license with associated degree required.
• CCM or CIRS credentials preferred.
• Must meet state licensure requirements.
• Experience in workers' compensation case management preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at the workplace or other sites as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide regular progress reports to clients and stakeholders.
• Ensure case management work meets regulatory and client requirements.
• Build strong relationships with clients, providers, and internal teams.
