Job summary

CVS Health's Aetna team seeks a Case Manager to coordinate complex care for dual eligible members. You will assess needs, plan and facilitate services, and advocate for options to promote quality outcomes.

Requirements include an IL residence, 3–5 years clinical experience, and 2–3 years in discharge planning or home health care coordination. Travel up to 50–75% and strong communication skills are essential.

Qualifications

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Confident working at home and able to collaborate virtually.
• Must have reliable transportation and travel up to 50-75% in specified areas.
• Excellent analytical and problem-solving skills.
• Effective communications and organizational skills.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.

Responsibilities

• Assessment of members to determine needs and plan care.
• Apply clinical guidelines and coordinate benefits to ensure suitable care.
• Collaborate with supervisors and Medical Directors to overcome barriers.
• Monitor plans of care and document outcomes in line with policies.
