You will collaborate with case managers and medical directors, monitor care plans, and empower members to participate actively in healthcare decisions while ensuring regulatory compliance.

Qualifications

• 2+ years in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% in specified Illinois counties.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate member needs/eligibility and propose case resolution strategies.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Consult with multidisciplinary teams to improve outcomes and address barriers.
• Escalate quality of care issues and advocate for member benefits.
