Job summary

CVS Health is seeking a Case Management Coordinator in Illinois. This full-time telework role involves 50-75% travel across DuPage, Kane, DeKalb and Kendall counties.

You will collaborate with care teams to schedule appointments, identify benefits, and guide members through education and resources to achieve better health outcomes. You will analyze member needs, coordinate care plans, monitor progress, and engage with teams to resolve barriers.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or appropriate related field equivalent to program focus.
• Must be willing and able to travel 50-75% of the time in DuPage, Kane, DeKalb, Kendall Counties, Illinois. Reliable transportation required. Mileage is reimbursed per our company expense reimbursement policy.
• Must have 2+ years of experience of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members and suggest care plan approaches based on benefit plans and programs.
• Identify high-risk factors and coordinate appropriate referrals and care planning.
• Coordinate and monitor assigned care plan activities and progress.
• Consult with care teams to improve quality of care and overcome barriers to goals.
• Identify and escalate quality issues through proper channels.
• Use negotiation to secure options and services for member needs.
• Engage members to motivate healthy lifestyle changes and empower informed decisions.
• Participate in team meetings and provide peer mentoring when needed.
• Assist members to participate actively with providers in decision-making.
• Document and monitor care using approved care management processes.
