Job summary

UnitedHealth Group is hiring a Care Coordinator to serve as the primary care manager for a panel of dual eligible members. This role supports medical, behavioral, and social needs, coordinating with care teams and community resources to promote appropriate service use and improved outcomes.

The position requires an active Idaho license (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT) and at least 1 year of Medicaid/Medicare or SDoH experience, plus proficiency in MS Office.

Qualifications

• Active state licensure in Idaho as one of listed credentials.
• Experience with Medicaid/Medicare or SDoH-focused care.
• Proficient with MS Office and ability to travel up to 50%.

Responsibilities

• Serve as the primary care manager for a panel of dual eligible members.
• Conduct comprehensive needs assessments including medical, behavioral, functional, cultural, and social drivers of health.
• Develop individualized, person-centered care plans and coordinate with internal care team and community resources.
• Assist members with obtaining HCBS supports and services.
• Provide referrals and linkage to internal and community resources as appropriate.
• Support discharge planning and care transition after ER, hospital, or SNF stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for members to ensure needs and choices are represented by the health care team.
• Support nonclinical questions with credentialing, claims, and related processes.
