Qualifications

• Idaho license or equivalent per listed options.
• 1+ year experience with Medicaid/Medicare or SDoH.
• 1+ year MS Office experience.
• Access to a home workspace with PHI security.
• Ability to travel within assigned territory up to 50%.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs and wellness assessments.
• Collaborate with care team, providers, and community resources to implement plans.
• Assist with obtaining HCBS supports and services.
• Provide referrals to internal and external resources as appropriate.
• Coordinate care transitions after ER visits or SNF stays.
• Educate and coach members for self-management of care.
