The position offers a 4x10 schedule option (7 am–6 pm) with 50–75% field time, remote flexibility for Mecklenburg County, NC and surrounding areas, and no on-call or weekend requirements. Strong communication and clinical skills are essential.

Qualifications

• Current, unrestricted RN licensure.
• 2+ years of clinical experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Willingness to travel up to 75% in the assigned territory.
• Reside in Mecklenburg County, NC, or surrounding areas.

Responsibilities

• Assess, plan and implement care management interventions for each member.
• Identify and initiate referrals for healthcare and community-based services.
• Develop and implement care plans as a single point of contact.
• Communicate required health information to ensure coordinated care.
• Advocate for members and families to meet needs and preferences.
