Qualifications

• Active and Unrestricted RN license in NJ.
• Residence in Union County, NJ.
• 75% travel to Hudson and surrounding counties in New Jersey.
• NJ Choice certification and state training modules completed.

Responsabilités

• Conduct face-to-face visits with members in their homes.
• Develop, implement and monitor care plans.
• Coordinate with PCPs and care team for appropriate services.
• Document accurately in the electronic health record.
• Mentor new hires after gaining proficiency.
