Job summary

Humana/iCare is seeking a Case Manager to join the GSR3 team in the Family Care Partnership program across Milwaukee and surrounding counties. The role provides care management through assessments, service plan development, and ongoing care coordination for adults with disabilities and the frail elderly.

You will coordinate resources, monitor progress toward outcomes, and collaborate with physicians to optimize member care.

Qualifications

Wisconsin Social Worker certification with 1+ year experience in Family Care target groups.
Bachelor’s degree in Human Services with 1+ year experience in Family Care target populations.
Bachelor’s degree in other areas with 3+ years experience in Family Care populations.
Knowledge of community resources and long-term care options.
Ability to travel within Milwaukee, WI counties for home assessments.

Responsibilities

Conduct psychosocial assessments as part of the comprehensive assessment process.
Coordinate and monitor services to meet identified outcomes.
Reassess and update care plans and monitor health and safety.
Assist member or representative in filing complaints or advocacy services.
Document member information and contacts per policy and procedures.
Build relationships with member, family, and outside agency staff.
