Job summary

Humana/iCare is seeking a Case Manager/ Care Coach to join the GSR3 team serving Milwaukee and surrounding counties in the Family Care Partnership program. The role includes assessment, care planning, coordination, and ongoing monitoring for adults with disabilities and the frail elderly.

Responsibilities include conducting psychosocial assessments, coordinating services, documenting care plans, and collaborating with families, physicians, and community resources.

Qualifications

• Social Worker certification in Wisconsin and experience with Family Care target populations.
• 4-year bachelor’s degree or higher in human services or related field with 1+ year experience.
• Knowledge of community resources and long-term care options.

Responsibilities

• Conduct psychosocial assessments as part of the comprehensive assessment process.
• Coordinate and monitor services to meet member outcomes and progress.
• Document member information and care coordination activities.
• Collaborate with member, family, and providers to coordinate care.
• Educate members and providers about benefits, services, and resources.
• Travel to member residences to conduct assessments as needed.
