Job summary

Humana Inc is seeking an organized case manager to serve Wisconsin Family Care members, coordinating with a Field Care Nurse to meet health and safety needs. You will conduct in-home assessments, quarterly visits, and regular phone contacts while arranging non-medical supports and maintaining thorough documentation.

The role requires a Bachelor's degree in human services or related field (or 3+ years in lieu) and a valid driver's license.

Qualifications

• Bachelor's degree in human services or related field with 1 year of relevant experience.
• Or Bachelor's degree in another field with at least 3 years of relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, usually at the member's home.
• Conduct quarterly in-person visits and monthly phone contact.
• Arrange non-medical support services including social integration, housing, and community resources.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain accurate documentation including case notes and MCP updates.
