Job summary

Humana/iCare in Wisconsin seeks a Case Manager to join the GSR3 team across Milwaukee, Racine, Kenosha, Walworth, Waukesha, Washington, Ozaukee, and Sheboygan counties in the Family Care Partnership program.

You will perform psychosocial assessments, coordinate services, develop and monitor care plans, and educate members and providers about benefits and resources to support adult clients with disabilities and the frail elderly.

Qualifications

• Wisconsin Social Worker certification required.
• At least 1 year of experience with Family Care populations.
• Bachelor’s degree in Human Services or related field.
• Willingness to travel up to 75% within Wisconsin counties.
• Knowledge of Medicaid/Medicare benefits.

Responsibilities

• Conduct psychosocial assessments as part of comprehensive assessment process.
• Coordinate and monitor services to meet member outcomes and progress.
• Assist member or representative in filing complaints and obtaining advocacy services.
• Document member information and contacts regarding care and services.
• Create documentation per policy, procedures, workflows and audits.
• Build constructive relationships with family and outside agency staff.
• Coordinate with member’s physician or medical professionals to coordinate care.
• Educate member and providers about benefits and resources.
• Monitor engagement in treatment and explore alternatives.

Skills

Wisconsin Social Worker certification

Bachelor’s degree in Human Services
