Job summary

Humana Inc. in Milwaukee, WI, seeks an RN Care Manager to join the Family Care Partnership (FCP) program and provide case management to adults in the community.

You will conduct home and community visits, complete nursing assessments, monitor wellness, coordinate services, and work with members, families, and outside agencies to ensure safe, effective care. Travel up to 50% within Milwaukee and surrounding counties may be required.

Qualifications

• Must be a Registered Nurse licensed in Wisconsin in good standing.
• Two years of related health care experience.
• BSN preferred.
• Experience in home health, managed care or case management preferred.

Responsibilities

• Complete nursing portion of comprehensive assessment and input to member care.
• Monitor wellness and health promotion activities based on assessment.
• Re-evaluate health and wellness at intervals to manage ongoing issues.
• Perform routine health monitoring activities as needed.
• Build relationships with members, families and outside agency staff.
• Assess safety of medication, medical care interventions and treatments.
• Provide or arrange services and monitor effectiveness.
• Assist with program development and input for QA/QI.
• Travel to Milwaukee and surrounding counties as needed.
