Humana/iCare is seeking a Case Manager to join the GSR3 team supporting adults with disabilities and frail elders through the Family Care Partnership (FCP) program. This remote role covers Milwaukee, Racine, Kenosha, Walworth, and surrounding counties.

Responsibilities include psychosocial assessments, care planning, coordination, advocacy, and documentation. Travel up to 75% is required to conduct assessments in members' homes and with providers. Competitive benefits accompany the role.

Qualifications

• Wisconsin Social Worker certification with at least 1 year of experience with Family Care target populations.
• Bachelor's degree (4-year) in Human Services or higher with at least 1 year of Family Care experience.
• Bachelor's degree in any field with at least 3 years of experience with Family Care populations.

Responsabilités

• Conduct psychosocial assessments as part of the comprehensive assessment process.
• Coordinate and monitor services to meet member outcomes and progress.
• Assist member or member's representative in filing complaints, grievances and advocacy.
• Document member information and contacts regarding care and services.
• Create documentation according to policy, workflows, audits and time frames.
• Build relationships with member and family, and with external agency staff.
• Coordinate with physicians or medical professionals to align member care.
• Educate members and providers about benefits, resources and care protocols.

Formation

Social Worker certification in Wisconsin

Bachelor's degree in Human Services or higher

Bachelor's degree in any field with 3+ years in Family Care
