Job summary

Humana/iCare is seeking a Case Manager to join the growing GSR3 team (Milwaukee, Racine, Kenosha, Walworth, Waukesha, Washington, Ozaukee, Sheboygan) in the Family Care Partnership (FCP) program.

The FCP Case Manager/Care Coach provides care management services through the FCP program including assessment, service plan development, ongoing care coordination, quality assurance and ongoing monitoring of services to adults with disabilities and the frail elderly.

Qualifications

• Wisconsin Social Work certification with 1+ year experience in Family Care target populations.
• Bachelor's in Human Services or higher with 1+ year Family Care experience; or non-human-services degree with 3 years Family Care experience.
• Knowledge of community resources and long-term care options; ability to travel regionally.

Responsibilities

• Conduct psychosocial assessments as part of the comprehensive assessment process.
• Coordinate and monitor services and resources to meet member outcomes.
• Develop and update care plans; monitor health and safety.
• Assist members or representatives with complaints, grievances, and advocacy.
• Document interactions and create documentation per policy and timelines.
• Facilitate collaboration with member and physicians; coordinate care with providers.
