Job summary

Humana/iCare seeks a Case Manager to join the GSR3 team across Milwaukee, Racine, Kenosha, and surrounding counties. You will assess, develop service plans, coordinate care, monitor progress, and advocate for members with disabilities and the frail elderly.

Responsibilities include reassessments, documentation, and collaboration with physicians and providers to optimize care. Travel up to 75% is expected; remote work is possible with field visits.

Qualifications

• Must be a Wisconsin-licensed or certified Social Worker.
• Bachelor's or higher in Human Services with relevant experience.
• Experience with Family Care target populations.
• Knowledge of local long-term care resources and options.
• Willingness to travel across multiple counties.

Responsibilities

• Conduct psychosocial assessments as part of the assessment process.
• Coordinate and monitor services to meet member outcomes.
• Document member information and care plan updates.
• Assist in filing complaints and obtaining advocacy services.
• Educate members and providers about benefits and resources.
