Job summary

Humana/iCare is seeking a Case Manager to join the GSR3 team in the Family Care Partnership program. The role involves assessment, service plan development, ongoing care coordination, and monitoring for adults with disabilities and frail elderly relatives across Milwaukee, Racine, Kenosha, and surrounding counties.

Responsibilities include comprehensive psychosocial assessments, resource coordination, and collaboration with physicians. Travel up to 75% and remote work options are available.

Qualifications

• Licensed Social Worker in Wisconsin or eligible for certification.
• 1+ year experience with Family Care target populations.
• Knowledge of Wisconsin community resources and long-term care options.

Responsibilities

• Conduct psychosocial assessments as part of the comprehensive assessment process.
• Utilize RAD methodology to identify outcomes and resources.
• Coordinate and monitor services to meet identified member outcomes.
• Periodically reassess and update the member care plan.
• Assist member or representative in filing complaints and advocacy.
• Document member information and care details per policy and timelines.
• Build relationships with member, family, and outside agencies.
• Coordinate with physician or medical professionals for care.
• Provide referrals to community resources and social services as needed.
• Monitor engagement and work with team to address needs.
