Job summary

Humana Inc in Wisconsin is seeking a compassionate case manager to serve Family Care members in Weston, WI, and surrounding areas. You will assess health and safety, develop a comprehensive Member Care Plan (MCP), and coordinate with a Field Care Nurse to deliver services in the least restrictive environment.

You will conduct enrollment assessments, regular visits, arrange non-medical supports, and maintain accurate documentation.

Qualifications

• Bachelor's degree in human services or related field is required with 1+ year experience serving frail elders or adults with disabilities.
• Alternatively, a Bachelor's in another field with 3+ years relevant experience is acceptable.
• Valid driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members' health and safety to develop a comprehensive Member Care Plan.
• Coordinate with a Field Care Nurse to provide services in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and maintain monthly contact by phone.
• Arrange non-medical supports including social integration, resources, housing, and employment.
• Maintain accurate MCP documentation and service authorizations.
