Job summary

Cognizant is seeking an Inpatient and Outpatient Facility Coder to review and assign accurate diagnosis and procedure codes across inpatient, outpatient, ED, ambulatory surgery, and related records. You will work with physicians and auditors to ensure regulatory compliance and quality outcomes.

The role requires AHIMA CCS or CPC certification, 3+ years of facility-based coding, and experience with ICD-10-CM/PCS, MS-DRG, CPT, and HCPCS.

Qualifications

• Certification as AHIMA CCS or CPC (AAPC) required.
• Minimum of three years of facility-based coding experience with ICD-10-CM, ICD-10-PCS, MS-DRG, CPT, and HCPCS.
• Level-1 Trauma coding experience required.
• Experience coding both inpatient and outpatient facility records.
• Knowledge of Federal, State, and payer regulations.
• Proficiency with Microsoft Office (Word, Excel, Outlook).

Responsibilities

• Review medical documentation and assign accurate diagnosis and procedure codes using ICD-10-CM, ICD-10-PCS, MS-DRG, CPT, and HCPCS.
• Interpret inpatient and outpatient records for coding accuracy and compliance.
• Communicate with physicians to obtain diagnoses and procedures via queries.
• Use electronic encoder application to code per policies and guidelines.
• Perform peer reviews of coding auditors and coders.
• Maintain coding accuracy and meet productivity expectations.
• Stay current with coding guidelines and reimbursement methodologies.
• Support quality improvement through coding reviews and education.
• Conduct other coding-related duties as assigned.
