Job summary

HOPE Family Health Services in Tennessee seeks a Community Health Worker to help patients access and navigate HOPE Family Health services and community resources. The CHW communicates with patients and care teams, identifies barriers to care, coordinates follow-up, and documents services accurately.

Depending on assignment, the CHW may focus on medical or behavioral health telephone support, language access, referrals, outreach, care coordination, or connecting patients with resources such as

Qualifications

• High school diploma or equivalent required.
• Experience in health care or patient services preferred.
• Excellent communication and cultural sensitivity.
• Familiarity with EHR systems and standard office software.
• Additional language skills or BLS may be required for certain assignments.

Responsibilities

• Build respectful, trusting relationships with patients and families.
• Contact patients by phone or in person; relay information and route concerns.
• Explain appointments, care plans, and next steps; reinforce instructions.
• Identify barriers to care and connect patients with HOPE programs.
• Coordinate with medical and behavioral health teams for continuity of care.
• Maintain accurate records in the electronic health record.
• Participate in team meetings and training.
