Job summary

Qualifications

Responsibilities

• Build trusting relationships with patients and families; support patient choice and self-advocacy.
• Contact patients by phone, message, or in person; relay information to care teams.
• Explain appointments and care plans; reinforce education without giving clinical advice.
• Identify barriers to care and connect patients with HOPE programs and resources.
• Coordinate with medical and behavioral health teams to support continuity of care.
• Maintain accurate records in the electronic health record; protect privacy.
• Participate in team meetings, training, and quality improvement activities.
