Job summary

Molina Healthcare seeks an Eligibility Screener for a remote, field-based role serving Wisconsin IRIS participants. You will visit participants in their homes and complete the Adult Long-Term Care Functional Screens, including annual rescreens and change-in-condition checks.

We value compassion, attention to detail, and strong organizational skills to ensure accurate screenings, timely data entry, and clear communication with care partners. Travel is expected around 5%.

Qualifications

• Bachelor's degree in health or human services or related field (e.g., social work, psychology).
• Experience with case management or home health is preferred.
• Proficiency with Microsoft Office Suite.

Responsibilities

• Meets with IRIS participants face to face to complete the screening process.
• Performs LTC-FS for IRIS participants according to Wisconsin guidelines and instructions.
• Verifies results with IRIS Consultants, Medicaid agencies, and physicians when needed.
• Maintains documentation and adheres to program regulations with timely completion.
