Job summary

TMG is seeking an Eligibility Screener to perform Adult Long-Term Care Functional Screen (LTC-FS) assessments for Wisconsin IRIS participants. This remote role involves visiting participants in their homes, conducting annual rescreens, and handling change-in-condition screens with compassion and integrity.

Ideal candidates have health/human services backgrounds, strong data accuracy, and excellent communication skills.

Qualifications

• Minimum 2 years in health care, preferably in care coordination and IRIS target groups.
• Must be able to complete LTC-FS certification modules within first week and maintain certification.
• Bachelor's or higher in health/human services; RN license active if nursing trained.

Responsibilities

• Meet with IRIS participants face to face to complete screening.
• Complete Adult Long-Term Care Functional Screen (LTC-FS) for IRIS participants.
• Verify results with IRIS Consultants, Medicaid Personal Care agencies, and physicians as needed.
• Maintain documentation and adhere to regulations; ensure timely screens.
• Participate in weekly team meetings and trainings; support other duties as assigned.
