Job summary

Pacific Health Group is seeking an ECM Program Manager to lead our Enhanced Care Management initiative across California. You will supervise Lead Care Managers, shape program quality, and partner with hospitals to improve care transitions and reduce readmissions.

The role requires strong leadership, data-driven evaluation, and a commitment to person-centered care and health equity. Travel across counties is expected, with remote work within California supported.

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• 5+ years in healthcare management, including 3+ years in care/case management.
• 2+ years managing Enhanced Care Management or intensive care management programs.
• Strong analytical skills and experience in data-driven program evaluation.

Responsibilities

• Lead Lead Care Managers across California counties.
• Oversee program quality, member services, and budget.
• Develop benchmarks, QA protocols, and standardized workflows.
• Collaborate with hospitals, providers, and community partners.
• Monitor outcomes and ensure regulatory standards.
• Pilot innovations such as telehealth to expand access.
