Humana Inc is seeking an E-Referral Specialist to join our remote team. You will be the first contact point for referrals, handling data collection, documentation, and follow-up to promote continuity of care.

You will monitor e-referral systems, manage communications, and determine patient eligibility while adhering to timelines and privacy standards. This role requires strong attention to detail and proficiency in Microsoft Office, with travel that may be required occasionally.

Qualifications

• High School Diploma or equivalent is required.
• Typing speed of 40 wpm or higher.
• Ability to understand and apply basic math to analyze data and generate reports.
• Proficiency with Word, Excel, Outlook, and Teams.
• Strong logical thinking and decision-making.
• Ability to adhere to strict timelines.
• Good communication with all levels of management.
• Ability to work with multiple screens/systems with accuracy.
• Strong time management in a fast-paced environment.
• Must be fluent in English (read, write, speak).
• Regular attendance is essential.
• Travel up to 0%.
• Experience with eReferral portals and EPIC is preferred.
• 1+ years in customer service or healthcare; home health experience is a plus.
• Knowledge of home health service delivery and Medicare enrollment is a plus.
• Associates degree preferred.

Responsibilities

• Monitor multiple e-referral systems for new referrals and manage alerts.
• Check emails for requests to pull referrals and follow up on communications.
• Screen documentation to determine patient eligibility for services.
• Track referral statuses and provide weekly status reports.
• Communicate with branch, sales, and leadership for updates and decisions.
• Maintain ERST log and ROC decisions daily.
• Maintain productivity as assigned and per standards.
• Ensure compliance with regulatory policies and procedures.
