Job summary

Humana Inc. is seeking an E-Referral Specialist to support CenterWell Home Health. You will manage referral intake, eligibility checks, and communications with branches and leadership to promote timely decisions.

This remote role requires fast-paced data processing, strict adherence to timelines, and strong proficiency with Microsoft Office. Occasional site travel may occur for training or meetings.

Qualifications

• High School Diploma or equivalent.
• Typing at least 40 words per minute.
• Proficient in Microsoft Word, Excel, Outlook, and Teams.
• Strong numerical reasoning and data reporting ability.
• Good communication across all management levels.
• Ability to manage multiple screens and systems with attention to detail.
• Able to work under strict deadlines.
• Excellent English reading/writing/speaking.
• Good attendance and reliability.
• 0% travel time; occasional office travel when needed.

Responsibilities

• Monitor multiple e-referral systems to identify new referrals and manage alerts.
• Monitor email for requests to pull referrals and follow up on communications.
• Scan documentation to determine patient eligibility for services.
• Monitor referral status and report progress weekly.
• Communicate with branches, sales, and leadership for status updates.
• Daily management of ERST log with referrals and ROC decisions.
• Maintain productivity levels per assignment and standards.
• Comply with regulatory policies and procedures.
