Job summary

Service Coordination, Inc. is hiring for a Community Service Coordinator to support individuals with developmental disabilities. You will advocate, educate families, collect eligibility information, and help coordinate services within the community.

Primary responsibilities include creating and updating Person Centered Plans, coordinating resources, and ensuring compliance with HIPAA and SCI policies. Travel within local areas is involved, with competitive pay and benefits.

Qualifications

Bachelor's degree in a human services field or related area as specified.
Experience in case management or service coordination preferred.
Knowledge of DDA services and eligibility processes is a plus.

Responsibilities

Advocate on behalf of individuals, provide resource information, and educate for self-determination.
Collect information and documentation for DDA eligibility and prioritize needs.
Plan and update Person Centered Plans (PCPs) and document outcomes.
Coordinate resources and services to meet PCP goals.
Monitor PCPs with timely follow-ups and evaluations.
Adhere to SCI policies, HIPAA, confidentiality, and safety procedures.
