Qualifications

• Education: Graduation from an accredited high school or GED.
• Experience: One year of professional experience coordinating health-related programs or referring people to resources.
• Substitutions: 30 college credits may substitute for experience; military service may substitute.

Responsibilities

• Coordinate services for individuals served by DDA through TCM and CCS programs.
• Assess and determine eligibility for DDA services and provide referrals.
• Arrange access to day, residential and other support services.
• Facilitate annual planning meetings and develop a person-centered plan (PCP).
• Monitor PCP and satisfaction with delivered services; manage waiting list transitions.
• Maintain accurate documentation of all services for quality and compliance.
