Job summary

Center for Human Services in Warsaw, MO is seeking a Service Coordinator to manage a caseload of individuals with developmental disabilities. You will build links to community resources, coordinate services, and document plans and outcomes with families and support teams.

The role requires a bachelor’s degree or RN license, strong communication and organizational skills, and the ability to work in a hybrid/remote setting within the service area. Monday–Friday daytime shifts.

Qualifications

• Requires a Bachelor’s degree from an accredited college or university or RN license.
• Must have reliable transportation, driving record, valid license, and internet access for telecommuting.
• Experience with case management and documentation tools is preferred.

Responsibilities

• Manage a caseload of varying ages and needs to improve support quality and independence.
• Develop community relationships and link resources to plan-based needs.
• Monitor and assess service quality and timeliness through regular contact and meetings.
• Document activities in case notes within three business days of each activity and maintain required hours.
• Provide up-to-date information to individuals supported and participate in ongoing training.
• Foster team unity and take on special assignments to support client goals.
