Job summary

Vynca Inc. is seeking a Director of Enhanced Care Management to lead the ECM program, shaping policy, operations, and staff development to improve health outcomes and cost of care.

You will supervise Clinical Managers and Lead Care Managers, coordinate with health plans, ensure data integrity and billing compliance, and drive staffing, training, and performance against SLAs, with quarterly travel.

Qualifications

• Bachelor's Degree required or equivalent experience.
• Masters in Social Work, Public Health, or Healthcare Administration highly preferred.
• 5+ years of people management experience in a similar environment.
• Remote workforce and/or multi-site supervisory experience required.
• 3+ years of care management experience; strong understanding of managed care programs (Medicare/Medicaid) highly preferred.
• Experience with Enhanced Care Management programs highly preferred.
• Proven results-driven business leadership with P&L responsibility.

Responsibilities

• Refine and implement program policies and procedures.
• Supervise Clinical Managers, Lead Care Managers, and Support Staff.
• Oversee day-to-day ECM program operations.
• Ensure ECM contract goals and SLAs are met.
• Oversee records, billing requirements, and data integrity.
• Oversee assessments, care planning, crisis management, and transition plans.
• Develop KPIs and align team performance with business objectives.
• Manage P&L and report results in monthly reviews.
• Collaborate with other department leaders to optimize referrals and patient onboarding.
