Qualifications

• Bachelor's degree and/or BSN required.
• 3+ years as a Certified Diabetes Educator (CDE) or CDCES.
• Experience with diabetes care and direct patient education.

Responsibilities

• Provide education about diabetes treatments and resources to help patients begin and manage therapy.
• Engage in one-on-one communications with enrolled patients via phone, video, email and SMS following program guidelines.
• Maintain accurate documentation and demonstrate strong interpersonal communication.
