Qualifications

• Requires degree from an accredited school of nursing or a BSN with 2–4 years of related experience.
• RN state licensure required; compact licensure accepted.
• Authorized to work in the U.S. without visa sponsorship now or in the future.

Responsabilités

• Evaluates member needs and barriers to care and develops person-centered plans.
• Develops ongoing care plans and collaborates with providers to identify resources.
• Identifies problems/barriers to care and initiates interventions.
• Coordinates between member/family and care team for appropriate services.
• Provides follow-up and monitoring to adjust care plans as needed.
• Provides resource support for local services based on assessments.
• Facilitates care management with providers to ensure timely access to care.
• May perform telephonic, digital, home, or on-site outreach to assess needs.
• Collects, documents, and maintains member information to ensure compliance.
• Delivers education to members and families on disease processes and care options.
• Provides feedback to leadership on opportunities to improve care delivery.
• Performs other duties and complies with policies.
