Job summary

Centene Corporation is seeking a Registered Nurse for a remote role based in Ohio, with in-person member visits within designated Ohio counties. The nurse will develop and coordinate complex care plans, address social determinants, and ensure high-quality, cost-effective outcomes.

Qualifications

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
• RN state licensure or compact licensure required.

Responsibilities

• Evaluates member needs and barriers to access.
• Develops care/service plans and coordinates with providers.
• Identifies problems and provides care management interventions.
• Coordinates between member, family, and care team for person-centered care.
• Performs follow-up and monitors changes toward goals.
• Provides information on local resources and services.
• Facilitates care management and ensures timely access to care.
• May conduct outreach via phone, digital means, or home visits.
• Maintains member data to comply with regulations.
• Provides education to members and families about conditions and care options.
• Offers feedback to leadership to improve care delivery.
