Job summary

Community Living Case Management, Inc. is seeking a Service Coordinator/Case Manager in Coos County to support individuals with I/DD through person-centered planning and ongoing case management.

You will coordinate with clients, families, providers, and brokerages to develop ISPs, participate in needs assessments, and monitor plans while ensuring safety and compliance within state timelines.

Qualifications

• Bachelor's degree in behavioral science, social science, or closely related field or equivalent experience.
• Experience with ODDS programs, rules, timelines, and documentation standards.
• Experience conducting assessments and maintaining documentation.

Responsibilities

• Coordinate with individuals, families, and providers to fulfill ODDS tasks (ONA, ISP).
• Visit clients in homes or community settings to ensure health and safety and address critical incidents.
• Document meticulously to ensure compliance with state regulations and timelines.
