Job summary

United Disabilities Services is seeking a Service Coordinator to manage waiver services for individuals with disabilities and older adults. The role involves developing person-centered plans and coordinating services to enhance independence and quality of life.

Responsibilities include in-home visits, travel to counties in the region, and collaboration with service providers. A bachelor’s degree or equivalent case management experience is required.

Qualifications

• Bachelor's degree in social work, psychology or related field (with practicum) OR at least 3 years of case management experience in social services/healthcare.

Responsibilities

• Develop person-centered service plans addressing identified needs, goals, interventions, and progress.
• Coordinate services for participants according to identified needs.
• Monitor authorized services via in-home visits, phone calls, planning meetings, and liaison with providers.
