Job summary

The Coordinating Center is seeking a full-time Coordinator of Community Services to manage targeted casework for individuals with intellectual/developmental disabilities. You will locate and access medical, social, habilitative, and housing supports while coordinating with a multidisciplinary team to ensure person- and family-centered care.

Qualifications

• Bachelor’s degree in a human service focus or equivalent experience required.
• Experience coordinating services for Medicaid/waiver or IDD strongly preferred.
• Competency in negotiation, advocacy, problem solving and relationship building.
• Strong writing and speaking communication skills and a reliable computer skill set.

Responsibilities

• Maintain a caseload of 30–40 clients with IDD.
• Advocate for clients and support self-advocacy in service planning.
• Develop Person-Centered Plans in line with COMAR regulations and agency policies.
• Identify and access services across medical, social, vocational and housing supports.
• Document services to meet LTSS billing standards and internal systems.
• Attend meetings and coordinate with interdisciplinary teams.
