You will coordinate with families, providers, and schools to empower self-determination and ensure services meet state requirements. Qualifications

• Bachelor's degree in behavioral or social sciences, or equivalent experience.
• One year of human services related experience.
• Associate's degree with two years of human services experience.
• Three years of human services related experience.
• Valid driver's license.

Responsibilities

• Coordinate with individuals, families, and providers to fulfill essential ODDS tasks.
• Develop and monitor Individual Support Plans (ISPs) and participate in Needs Assessments (ONA).
• Visit clients in homes or community settings to ensure health and safety.
• Maintain accurate documentation and ensure compliance with state timelines.
• Advocate for clients while maintaining professional boundaries.
