• Support member-related correspondence and educational materials.
• Document and maintain community resources per health plan requirements.
• Assist the clinical team of nurses and social workers.
• Conduct non-clinical health assessments and refer to appropriate care/services.
• Assess home safety, environment resources, transportation, and employment for referrals.
• Outreach to locate hard-to-reach individuals and families.
• May visit homes or community organizations.
