Job summary

Reema Health, Inc. is seeking a Community Health Navigator to build relationships with members, assess needs, and guide them toward better engagement in their own healthcare. The role involves remote work with field-based travel up to 50% and a 25–50% travel requirement within a 30‑mile community service area.

You will document care plans, schedule visits, and use Reema's technology daily while collaborating with a multidisciplinary team to address social barriers and improve member wellbeing.

Qualifications

• Must have a solutions-oriented approach to overcome barriers and help members progress.
• Strong communication skills, both verbal and written, to describe problems clearly.
• Tech literacy to learn and troubleshoot new tools on mobile and desktop.
• Self-motivated to stay productive on a remote caseload without ongoing supervision.
• Ability to collaborate with a multidisciplinary team and participate in occasional in-person meetings.

Responsibilities

• Conduct outreach by phone and in person; enroll members in Reema's care model.
• Manage a caseload of 80-120 members; cover for other Guides as needed.
• Assess member needs, including social determinants of health, to plan care.
