Centene is seeking a remote-friendly role within Missouri to provide telephonic support to foster care members, connecting them with community resources and assisting the care team. The position runs Monday–Friday, 8am–5pm, and may involve some home visits or in-person outreach as needed.

Requires a High School diploma or GED and 1–2 years of related experience. Centene emphasizes comprehensive benefits and flexible remote work within the state.

Aufgaben

Provides support to members to connect them to known community and care resources.
Supports coordination of community outreach resources and promotes awareness of care/services.
Assists members with community and care resource inquiries and opportunities.
Supports all member-related correspondence and educational materials to assist in facilitation of community connection.
Documents and maintains all community resources per health plan requirements.
Assists clinical team with outreach, community education, and member support.
Performs non-clinical health assessments to refer members to appropriate care/services.
Performs non-medical assessments such as home safety, transportation, and community resources.
Conducts telephonic and/or in-person outreach to locate hard-to-reach individuals and families.
May visit homes or community organizations.
Has working knowledge of Social Determinants of Health (SDOH) barriers.
Performs other duties as assigned.
