Job summary

Centene Corporation is seeking a Community Connections Specialist to support members by connecting them to community resources and care services, helping manage their care journey with cost-effective resources.

The role offers remote work from anywhere in Missouri, telephonic support to foster care members, and a Monday–Friday 8am–5pm schedule. The position emphasizes strong communication and knowledge of social determinants of health.

Qualifications

• Requires a High School diploma or GED.
• Requires 1–2 years of related experience.

Responsibilities

• Provide telephonic support to foster care members and connect them to community resources.
• Coordinate outreach resources and promote awareness of care/services.
• Assist the clinical team with outreach, education, and member support.
• Document and maintain community resources per health plan requirements.
• Conduct non-clinical health assessments to refer members to care/services.
• Perform home safety and environmental resource assessments for referrals.
• Reach hard-to-locate individuals and families via telephonic or in-person outreach.
