Job summary

Reema Health is seeking a Community Health Navigator to engage with members and guide them toward greater healthcare engagement. You will work through Reema's technology platform daily to reach members, schedule visits, and document progress.

The role involves 25–50% travel within a 30-mile community service area and occasional evenings/weekends, with a focus on building relationships and addressing social determinants of health to improve wellbeing.

Qualifications

• Strong communication skills, both verbal and written.
• Comfort with learning and using new tools on mobile and desktop.
• Self-motivated with the ability to stay productive on a remote caseload.
• Ability to collaborate and build team connections in a remote environment.
• Proactive problem solving to navigate member barriers.

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members) and cover for others as needed.
• Assess member needs to create member-centered care plans.
• Establish trusting relationships and follow through to completion.
• Coordinate referrals and schedule attendance.
• Document all work thoroughly in the Reema App and maintain HIPAA compliance.
