Job summary

Reema Health, Inc. seeks a Community Health Navigator to build genuine relationships with members and guide them toward greater engagement in their own healthcare. You’ll work with members to address social barriers, schedule visits, and document activities in the Reema App while coordinating with a multidisciplinary team.

This role involves 25-50% travel within a 30 mile area, with a Monday–Friday schedule and occasional evenings or weekends in a fast-paced startup environment.

Qualifications

• Must be 18 years or older and able to work Monday-Friday core hours.
• Ability to work with a remote caseload and use technology daily.
• Experience in crisis planning, case management, and boundaries preferred.

Responsibilities

• Outreach by phone and in-person to enroll members in the care model.
• Manage a caseload of 80-120 members and coordinate referrals.
• Build member-centered care plans addressing social determinants of health.
• Document work thoroughly in the Reema App and maintain confidentiality.
