Job summary

Reema Health in Indianapolis seeks a Community Health Navigator to engage members, guide them to care resources, and document progress in the Reema App. This role emphasizes empathy, timely follow-through, and data integrity while supporting a growing startup.

You will collaborate with a multidisciplinary team, manage a remote caseload, and adapt to evolving program needs, with strong emphasis on confidentiality and community resource linkage.

Qualifications

• Strong verbal and written communication skills.
• Comfort learning and troubleshooting new tools on mobile and desktop.
• Self-starting and able to stay productive on a remote caseload.
• Collaborate to foster team connections, including occasional in-person meetings.

Responsibilities

• Conduct cold outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members) and cover for others as needed.
• Assess member needs and social determinants to build care plans.
• Establish trusting relationships and follow through in a timely manner.
• Explain the importance of health activities and help members overcome hesitations.
