Job summary

Reema Health in Indianapolis, IN seeks a Community Health Navigator to build relationships with members and guide them toward greater engagement in their healthcare. You will work remotely or in the field, using our platform to reach members, schedule visits, and document progress while addressing social barriers.

Qualifications

• 18 years of age or older.
• Available to work Monday-Friday during core business hours.
• Field-based role requiring travel 25-50% in 30 mi community service area.
• Valid driver's license, insurance, and reliable transportation.
• Pre-employment drug screening and background check.

Responsibilities

• Conduct outreach and enroll members into Reema's care model.
• Manage caseload of 80-120 members with coverage during absences.
• Assess needs, including social determinants of health, to build care plans.
• Coordinate referrals and schedule appointments for members.
• Document work in the Reema App and maintain HIPAA confidentiality.
